INFECCIONES POSTQUIRURGICAS DE HERIDAS EN MIEMBROS INFERIORES EN PACIENTES ADULTOS ATENDIDOS EN UN HOSPITAL GENERAL DE QUITO-ECUADOR.

Introducción: La infección postquirúrgica de una herida en miembros inferiores son complicaciones que ha constituido un hecho inseparable a la práctica quirúrgica desde sus comienzos hasta la actualidad, es la tercera más frecuente de la Infección Asociada a la Atención en Salud (IAAS) y la primera entre pacientes quirúrgicos. Objetivo: Determinar la frecuencia de las infecciones postquirúrgicas de heridas en miembros inferiores en un hospital general en Quito-Ecuador. Métodos: El diseño del estudio fue descriptivo-no experimental, prospectivo y transversal, con una muestra de 190 pacientes seleccionados bajo criterios de inclusión y exclusión en el servicio de cirugía de un hospital general y a los cuales se les solicitó la firma del consentimiento informado. Resultados: Se encontró una frecuencia significativamente incrementada (p<0,05) de la población masculina (n:115/60,5%) sobre la femenina (n:75/39,5%) y de la etnia mestiza (62,6%) sobre el resto. La edad promedio fue de 64±12 años. La prevalencia de infecciones postquirúrgicas de las heridas de miembros inferiores fue de 37,9% y de éstos el 52,8% (p<0,05) presentaron infección dentro de los 4 primeros días del postoperatorio. Conclusión:  un número reducido de pacientes presentaron infecciones postquirúrgicas, siendo la mayoría, infecciones superficiales de la incisión, lo que sugiere el cumplimiento de los protocolos de preparación de pacientes prequirúrgicos y de seguridad de la cirugía en el centro dispensador de salud.   Palabras clave: Infección, cirugía, miembros inferiores, heridas ABSTRACT Introduction: The post-surgical infection of a wound in the lower limbs are complications that have been an inseparable fact to the surgical practice from its beginnings to the present, it is the third most frequent of the Infection Associated with Health Care (IAAS) and the first among surgical patients. Objective: To determine the frequency of postoperative wound infections in the lower limbs in a general hospital in Quito-Ecuador. Methods: The study design was descriptive-non-experimental, prospective and cross-sectional, with a sample of 190 patients selected under inclusion and exclusion criteria in the surgery service of a general hospital and who were asked to sign the informed consent. Results: A significantly increased frequency (p<0.05) was found for the male population (n:115/60.5%) over the female population (n:75/39.5%) and for the mestizo ethnic group (62, 6%) over the rest. The average age was 64±12 years. The prevalence of postoperative infections of the lower limb wounds was 37.9% and of these, 52.8% (p<0.05) presented infection within the first 4 postoperative days. Conclusion: a small number of patients presented post-surgical infections, the majority being superficial infections of the incision, which suggests compliance with the pre-surgical patient preparation and surgery safety protocols in the healthcare provider center.   Keywords: Infection, surgery, lower limbs, wounds.
