description Introduction: Cervical ectopic pregnancy is defined as the implantation of the blastocyst outside the uterine cavity at the level of the cervix. It is an unusual condition that represents less than 0.1% of all extrauterine pregnancies. Its diagnosis requires ultrasonography images and laboratory results, in addition to clinical suspicion of ectopic pregnancy and requires rapid and effective intervention as it can result in an obstetric emergency. Objective: Describe a case of cervical ectopic pregnancy, its risk factors, clinical picture, diagnosis and treatment options. Case presentation: The case of a 34-year-old patient with a diagnosis of cervical ectopic pregnancy, who wishes to preserve her fertility, is presented, in whom clinical management with intramuscular methotrexate is performed at an early stage of pregnancy. Discussion: This case shows conservative management with pharmacological therapy, however, as the literature mentions, failure with drug treatment with methotrexate occurs and must be complemented with conservative surgical management. Conclusions: It was shown that conservative treatment in this type of pregnancies is favorable and avoids complex procedures that put the woman's reproductive life at risk.

Es una condición inusual que representa menos del 0.1% del total de embarazos extrauterinos. Su diagnóstico requiere de imágenes de ultrasonografía y resultados de laboratorio, además de la sospecha clínica de embarazo ectópico y requiere una intervención rápida y eficaz ya que puede resultar en una emergencia obstétrica. Objetivo: Describir un caso de embarazo ectópico cervical, sus factores de riesgo, cuadro clínico, diagnóstico y opciones de tratamiento. Presentación del caso: Se presenta el caso de una paciente de 34 años con diagnóstico de embarazo ectópico cervical, quien desea preservar su fertilidad, en la que se realiza un manejo clínico con metotrexato intramuscular en una etapa temprana del embarazo. Discusión: El presente caso muestra el manejo conservador con terapia farmacológica, sin embargo, como la literatura menciona, el fracaso con tratamiento medicamentoso con metotrexato sucede y se debe complementar con manejo quirúrgico conservador.
