| Achoimre: | The Constitution of the Republic of Ecuador in 2008 stipulates a restructuring of the health model in the country. As a result, the creation of a Nacional Heath System, constituted by two major pillars in the health institutions; the Red Pública Integral de Salud (RPIS) and Red Privada Complementaria (RPC). These two large health networks interact, articulate and maintain a publicprivate relationship of health benefits, with the sole purpose of guaranteeing access, adequate care, and the right of health to those who need it. Once the benefit is provided, and by complying with the requirements stipulated in the relationship norm, the health care provider requests to the Coordinación Zonal de Salud to which it belongs, the cancellation of the values corresponding to the payment for the services rendered. The present study deals with one of the components of health benefits, the carrying out of complementary imaging testing, performed between January and June of 2016 by RPC providers belonging to the Coordinación Zonal 9 - Salud. The files physically rest in the archive department, and they are digitally recorded in the platform of the Registro Archivo Control (RAC) of the institution. A total of 3064 imaging services were reviewed, resulting in a total of 3713 examinations performed, of which 59 percent corresponded to magnetic resonance imaging, 19 percent to computerized axial tomography, 15 percent to radiography, 5 percent to special tests and finally 2 percent ultrasounds. The results of the study correspond to the determination of the average cost of the examinations performed, being 303.74 USD per test and 368.08 USD per patient. Overall, during the time of the study, a total of 1,127,798.25 USD were requested for the settlement of these benefits. For the reduction of unnecessary benefits, duplicated or benefits that should not be covered by MSP, it is proposed the implementation of control measures prior the referral of patients. For future studies it could be consider the comparison of costs between purchasing services from private institutions and those conducting examinations in public health institutions. |
