| Shrnutí: | The discharge plan is a protocol used by health personnel to provide instructions to the patient or family about the care they should perform at home; The activities involve guidance on the administration of medication regimens in order to avoid readmissions, especially in vulnerable populations such as children under five years of age. The objective of this study was to evaluate the nursing discharge plan in children under 5 years of age with respiratory diseases. Liborio Panchana Sotomayor General Hospital. Saint Helena, Period. January – April 2023. The methodology used was descriptive, exploratory, non-experimental, quantitative, the sample of children was 30 and nursing staff was 8, as an instrument a survey was used for the parents of the minors and a Check List for nurses. The results showed that those under 5 years of age were distributed in 60% women and 40% men, those who were 1 year old were 30%; The caregivers were 77% women, 60% aged between 20 and 29 years; The hours requiring care was 24 hours in 74% of the cases, with total dependency in 50% of the sample; The most prevalent diagnosis was pneumonia in 60% of the cases; The nurses were between 36 and 40 years old in 50% of the sample. The main deficits were non-use of discharge plan forms in 100%, non-identification of the user by age in 36%, and no explanation of patient care. skin in 24% and no explanation of seizure care in 24%. It was concluded that it is important to have a written discharge plan form to cover the main needs of knowledge and attitude of caring for children under five years of age with respiratory diseases.
