| description | Medication reconciliation is a process that consists of detailing and comparing each of the medications that the patient usually takes with the one prescribed during admission to a hospital unit or before a transfer, with the purpose of resolving discrepancies found and preventing errors. in medication or adverse events, for this reason the objective of this research work was to evaluate the application of the medication reconciliation protocol in patients with chronic pathologies in the internal medicine service of the Alfredo Noboa Montenegro Hospital January 2022 - April 2023, using a descriptive, observational, retrospective, cross-sectional, documentary and field methodology, the study population was 155 medical records and 10 nursing professionals from the hospital's internal medicine service, a sample with which the entire research project was developed, as technique and data collection was an instrument for the audit of the clinical history and a survey of 17 questions applied to the nursing professional, the results were 100% comply with the reconciliation of medications in patients with chronic pathologies, but there are deficiencies when to knowledge and when the most prevalent chronic diseases are arterial hypertension with 44%, diabetes mellitus with 29% and therefore COPD with 16%. Keywords: Medication reconciliation, chronic patients, arterial hypertension, mellitus diabetes.La conciliación de medicamentos es un proceso que consiste en detallar y comparar cada uno de los medicamentos que el paciente toma habitualmente con la prescrita durante el ingreso a una unidad hospitalaria o antes de un traslado, teniendo cuyo propósito es resolver las discrepancias encontradas y prevenir errores en la medicación o eventos adversos, por tal motivo el presente trabajo de investigación tuvo como objetivo evaluar la aplicación del protocolo de conciliación de medicamentos en pacientes con patologías crónicas en el servicio de medicina interna del Hospital Alfredo Noboa Montenegro enero 2022 – abril 2023, usando una metodología descriptiva, observacional, retrospectiva, transversal, documental y de campo, la población de estudio fue de 155 historias clínicas y 10 profesionales de enfermería del servicio de medicina interna del hospital, muestra con la que se desarrolló todo el proyecto de investigación, como técnica y recolección de datos fue un instrumento para la auditoria del historial clínica y una encuesta de 17 preguntas aplicada al profesional de enfermería, los resultados fueron el 100% cumple con la conciliación de medicamentos en los pacientes con patologías crónicas, pero existe deficiencias en cuando al conocimiento y en cuando a las enfermedades crónicas con más prevalencia son la hipertensión arterial con el 44%, la diabetes mellitus con un 29% y por ende el EPOC con un 16%.
