Para el desarrollo del mismo se usó la metodología Cascada, por ser un modelo de desarrollo simple y de fácil ejecución para proyectos de corto plazo y con requerimientos ya definidos. Se utilizó herramientas de software Open Source para el desarrollo del proyecto, cumpliendo con el decreto N° 1014 de Software Libre. Se muestra un informe de pruebas que el proyecto cumple con los requerimientos establecidos, el uso de esta herramienta informática contribuye de manera favorable porque permite registrar la información del formulario de una manera eficiente, logrando reducir recursos de tiempo y suministros de oficina para cumplir con la mejora de la calidad de atención de pacientes.The present project has as finality to perform one systematization of Form 002 outpatient provided by the Ministry of Health because the processes involve takes them manually making it much harder to manage information, patient care and the time to perform these activities It is very long to do it by the staff of the various health units; for all these reasons the need to create a system to manage patient records allowing easier and faster handling of information of medical care the patient was found. For the development of the waterfall methodology was used, as a model of simple and easy to implement for short-term projects already defined requirements and development. Open Source software tools were used for the project, complying with the decree N° 1014 free software.
