ROBERTO GILBERT ELIZALDECANTON GUAYAQUILECUADORPDFAntecedentes: A nivel mundial y regional las malformaciones ano-rectales toman lugar en uno de cada cinco mil nacidos vivos, siendo el ano anterior (o fístula perineal) el tipo más frecuente. La decisión de atender esta patología mediante anoplastía o colostomía en tres tiempos yace en la discreción clínica. Objetivo: Establecer la utilidad de la colostomía en tres tiempos vs. anoplastia en el manejo inicial del ano anterior – fístula perineal en término de eventos adversos en una población lactante. Metodología: Estudio descriptivo, analítico, retrospectivo. Se incluyo en el estudio lactantes atendidos por el servicio de Cirugía Pediátrica del HRGE entre agosto del 2016 a diciembre del 2020. Resultados: Se recuperó 63 casos, de los cuales 24 fueron operados mediante colostomía en tres tiempos y 39 mediante anoplastia, de los cuales: 22/22 (91.7%) y 21/39 (53.8%; P=0.0043) no se realizaron dilataciones post-quirúrgicas; 10/22 (41.7%) y 37/39 (94.9%) presentaron eventos adversos, respectivamente. Conclusión: La colostomía en tres tiempos presenta menos eventos adversos en comparación con la anoplastia. En el presente estudio no se distinguió otros factores asociados para con el desarrollo de eventos adversos. Sin embargo, es importante recalcar la significativa menor asistencia a dilataciones post-cirugía entre quienes se les realizó anoplastia. The anterior anus (or perineal fistula) is the most frequent type. The decision to treat this pathology through anoplasty or three-stage colostomy lies in clinical discretion. At the moment, no local experience has been documented in Ecuador, mainly with respect to secondary adverse events of both procedures.Aim: Establish the usefulness of the three-stage colostomy vs. anoplasty in the initial management of the anterior anus - perineal fistula in terms of adverse events in an infant population. Methods: Observational, analytical, longitudinal prospective study, of prospective recovery, of the cohort type, to be carried out in infants cared for by the Pediatric Surgery service from the HRGE, between August 2016 and December 2020. Results: Sixty-three cases were recovered, of which 24 underwent three-stage colostomy and 39 through anoplasty, of which: 22/22 (91.7%) and 21/39 (53.8%; P = 0.0043), no post-dilatations were performed. surgical; 10/22 (41.7%) and 37/39 (94.9%) presented adverse events, respectively. Conclusion: The three-stage colostomy has fewer adverse events compared to anoplasty. In the present study, other factors associated with the development of adverse events were not distinguished.
