Objetivo: Analizar la evidencia científica sobre el tratamiento quirúrgico periodontal versus terapia periodontal básica. Metodología: La presente investigación es de tipo documental, descriptiva, analítica y retrospectiva en su búsqueda, presenta un método deductivo; a partir de las palabras clave “enfermedad periodontal, terapia básica periodontal, terapia periodontal quirúrgica”, se revisaron 100 artículos científicos desde el 2016 hasta 2021, que fueron obtenidos en la web gracias a los buscadores Google Académico, Scielo, Redalyc, y en los metabuscadores PubMed, Web of Science, Cochrane, de los cuales, en base a criterios de inclusión y exclusión se analizaron 56 artículos. Resultados: La terapia periodontal básica permite reducir los niveles de los factores proinflamatorios del líquido crevicular, lo que reduce la inflamación que caracteriza a las enfermedades periodontales menos agresivas; sin embargo, en las enfermedades más agresivas como la periodontitis se debe tener en cuenta el tratamiento quirúrgico. Conclusión: Al describir los protocolos de la terapia periodontal básica y del tratamiento periodontal quirúrgico, se obtuvo una mejor comprensión de la forma en que deben emplearse estos tratamientos, y cuál es el tratamiento adecuado para cada enfermedad, según la actual clasificación.Introduction: Periodontal diseases are infectious and inflammatory disorders that affect the supporting tissues of the teeth, so making a correct diagnosis will allow us to define the best treatment option for each patient, since each individual may present a different alteration mediated by a systemic disease or some risk factor. Objective: To analyze the scientific evidence on periodontal surgical treatment versus basic periodontal therapy. Methodology: This research is documentary, descriptive, analytical and retrospective in its search, it presents a deductive method; from the keywords "periodontal disease, basic periodontal therapy, surgical periodontal therapy", 100 scientific articles were reviewed from 2016 to 2021, which were obtained on the web thanks to the search engines Google Scholar, Scielo, RedALyC, and in the meta search engines PubMed, Web of Science, Cochrane, of which, based on inclusion and exclusion criteria, 55 articles were analyzed. Results: Basic periodontal therapy reduces the levels of pro-inflammatory factors in the crevicular fluid, which reduces the inflammation that characterizes less aggressive periodontal diseases; however, in more aggressive diseases such as periodontitis, surgical treatment should be considered. Conclusion: In order to describe the protocols of basic periodontal therapy and surgical periodontal treatment, a better understanding was obtained of how these treatments should be used, and what is the appropriate treatment for each disease, according to the current classification.Universidad de Guayaquil.
