Su etiología aún no es exacta pero la teoría más aceptada por la mayoría de los autores es por la hiperactividad de la lámina dental. Los dientes supernumerarios pueden estar asociados o no a algún síndrome y su presencia puede llevar a la formación de alguna patología como diastemas, apiñamiento dental, quistes, maloclusión, entre otras. El cuarto molar o distomolar representa del 15% al 26% de todos los dientes supernumerarios y es el segundo más frecuente. En la literatura no existe una técnica específica para su tratamiento, ya que cada paciente tiene casos específicos y requerirán diferentes técnicas para su tratamiento. Para este estudio se indagó el tema de las piezas supernumerarias en varias bibliografías científicas, y la realización de un caso clínico empleando el conocimiento teórico práctico obtenido. Se presenta a la consulta un paciente masculino de 26 años de edad en la facultad piloto de odontología de la Universidad de Guayaquil, donde se realizó un examen radiográfico encontrándose una pieza supernumeraria retenida distal de la pieza 28. Se realizó el tratamiento quirúrgico para la extracción de ambas piezas y se le dio indicaciones postoperatorias. Se lo citó una semana después para comprobar que no existan anomalías en la recuperación de los tejidos.Supernumerary teeth are abnormalities that consist on dental surplus of any dentition and occur more frequently in men than in women; the diagnosis is made through panoramic radiography. Its etiology is not very precise yet but the hyperactivity of the dental lamina is the most accepted theory by majority of authors. The supernumerary teeth may be associated or not with a syndrome and its presence can lead to the formation of some pathology like diastema, dental crowding, cysts, malocclusion, among others. The fourth molar or distomolar represents the 15% to 26% of all supernumerary teeth and is the second most frequent. In literature there is no specific technique for its treatment since each patient will present specific cases and will require different techniques for their treatment. For the purpose of this paper, a research was made on the topic of supernumerary pieces in several scientific bibliographies, and the realization of a clinical case using the theoretical and practical knowledge obtained. A 26 year old male patient showed up for a consultation at the Dental School of the University of Guayaquil, where a radiographic examination was performed on him. The results showed a distal retained supernumerary piece located after the piece 28. Surgical treatment was performed to remove both pieces and the patient was given post operatory instructions. A final consultation was scheduled a week later to check that there were no abnormalities in the patient`s tissue recovery.Universidad de Guayaquil.
