El tercer molar presenta su erupción a una edad media de 19,5 a 20,5 años, es por tanto, el último diente en erupcionar, por lo que fácilmente puede quedar impactado o sufrir desplazamientos, si no hay espacio suficiente en la arcada dentaria, no evolucionan hacia una situación correcta, pudiendo generar patología. Para comprender esto debemos conocer las condiciones embriológicas y anatómicas de estos dientes. Topográficamente, esta pieza dentaria se ubica en un sector denominado Región del Tercer Molar Inferior, que tiene como límite anterior, una línea paralela a la cara distal del segundo molar, que se prolonga hasta el borde inferior de dicho hueso maxilar, un límite superior, que se origina en la cara triturante del segundo molar inferior y se extiende hasta su intersección con el límite posterior, un plano posterior que pasa 1cm por detrás de la porción más distal del tercer molar inferior y un inferior que es paralelo al superior y coincide con el punto más inferior de dicha pieza dentaria. El límite externo dado por la porción vestibular del hueso maxilar y el interno del mismo modo por la cara lingual de dicho hueso. Esta pieza dentaria en un alto porcentaje de situaciones no erupciona normalmente, es decir que se encuentra total o parcialmente retenida en el hueso maxilar, ocasionando patologías, como formaciones de quistes o 2 neoplasias, apiñamientos dentarios, infecciones agudas pericoronaritis y úlceras capuchones, complicaciones del nervio. Dentario inferior, parestesias, neuritis, entre otras.
