El presente estudio de investigación tiene como objetivo determinar la eficacia de la incisión semilunar modificada en cirugía de premolar maxilar retenido, la técnica de incisión será aplicada para la exodoncia de esta pieza retenida. A continuación, reportamos un caso de un segundo premolar maxilar retenido en posición transversal se utiliza en su tratamiento quirúrgico con una técnica de incisión semilunar modificada con un abordaje palatino. Paciente de 13 años de sexo femenino, se realizó un diagnóstico previo se logra observar ausencia de la pieza 25 y perdida de espacio para la erupción de la misma, no presenta sintomatología. Estudios imagenológicos previos se pudo determinar la vía de acceso por palatino, su densidad ósea y su morfología dentaria. Utilizando una tomografía computarizada observando la presencia de un segundo premolar maxilar retenido en posición transversal, la corona dirigida hacia palatino y la raíz hacia vestibular a nivel de los ápices de las piezas 24 y 26. Se logro realizar la cirugía sin ningún tipo de complicaciones transoperatorias. Su evolución al cabo de las primeras 24 horas no presenta edema, ni inflamación, la técnica utilizada dio excelentes resultados postoperatorios, sin complicaciones y pronostico favorable. Ayudándonos también de la medicación postoperatoria y cuidados, tuvimos excelentes resultados. La evaluación posterior a diez y veinte días, presenta una buena cicatrización, coloración de la mucosa palatina y no presenta ninguna anomalía en el proceso de cicatrización.One of the pathologies with a low incidence in oral surgery is the presence of a second retained upper premolar.
