| Shrnutí: | Alzheimer's disease (AD) is a neurodegenerative condition of multifactorial origin, characterized by the progressive loss of intellectual functions such as: memory, judgment, orientation, behavior and language, whose evolution generates dependence on vital activities. Due to its consequences and challenges for the caregiver, the present bibliographic review was carried out with the objective of establishing the assessment, using the guidelines of the Roper, Logan and Tierney model and proposing a care plan aimed at the elderly with AD, articulating the proposed model and the NANDA-NOC and NIC taxonomy. The information obtained from 73 selected bibliographic documents, which published in the scientific bases: SciELO, ProQuest, Virtual Health Library, E-books, academic Google, published from 2015 to 2020, which allowed the realization of the triangulation and analysis. The information organized according to the vital activities of the Roper-Logan-Tierney model; the care plan established. Finally, a care plan proposed, according to the interventions suggested by the authors and it was articulated according to the standardized language of the Nursing Interventions Classification (NIC), among which the following stand out: dementia management; help with self-care: dressing / feeding / hygiene; environmental management: security; Emotional Support; stimulation of family integrity; improve coping and sleep |
