| Summary: | ABSTRACT: Refractory epilepsy in pediatrics presents a significant therapeutic challenge. Against this backdrop, the ketogenic diet has emerged as a viable treatment alternative. This research aimed to update the scientific information concerning the most appropriate management of refractory epilepsy in pediatrics through the ketogenic diet. A basic, descriptive, and crosssectional study was conducted, which included a bibliographic review of 33 documents published in the last five years. These were sourced from regional databases such as Latindex, Lilacs, Scielo and high-impact databases like Scopus, Medline, and PubMed. The key findings highlight new updates on the effectiveness of the ketogenic diet, which induces a state of ketosis that, in turn, helps to reduce the frequency and severity of seizures. Advances in the adaptability of the diet based on the patient's age, the presence of comorbidities, and the type of epilepsy were also emphasized. Moreover, it was demonstrated that the ketogenic diet could be beneficial not only as a therapeutic option but also for enhancing the quality of life of affected children. In conclusion, the ketogenic diet has gained traction as an effective alternative for managing refractory epilepsy in pediatrics. This review highlights the need for more updated guidelines and underscores the importance of customizing dietary interventions. Healthcare professionals must stay abreast of these developments to offer the best treatment options to this vulnerable population. |
