Sumari:ABSTRACT: Acute appendicitis is the most common abdominal surgical emergency around the world, with perforation rates reaching 70% in elderly patients; Once this event has occurred, it is a matter of time for the infection to overcome the individual's defense mechanisms and spread to the peritoneal cavity, causing the so-called peritonitis due to appendicitis, increasing the risk of death due to the septic process. The definitive treatment consists of eliminating the source of infection itself, which can be done by open surgery or laparoscopy. The objective of this work was to update the scientific information on the approach that has presented the most advantages and fewer complications in the surgical treatment of appendiceal peritonitis. For this, a basic, descriptive, and transversal research was carried out, which consists of a bibliographic review of scientific documents published in the last 5 years. 52 documents were identified that were used in the study. The information was obtained from both regional (Latindex, Lilacs, Scielo, Redalyc) and high-impact (Scopus, PubMed, Medline, Web of Science) scientific databases. Among the main results, the understanding of the clinical manifestations, causes and etiopathogenic mechanisms, the description of the surgical approaches, as well as the presentation of advantages, disadvantages, and complications of each approach as definitive surgical treatment stand out. It is concluded that both approaches are safe and effective for the surgical treatment of appendiceal peritonitis; however, the multiple advantages of laparoscopy make this a procedure that leads to a reduction in the incidence of postoperative complications, being feasible and effective despite this. It is undeniable that the approach will be the surgeon's choice
