Резюме:The present study, titled “Update on the Clinical, Diagnostic, and Therapeutic Presentation of Premature Rupture of Membranes in Pregnancy,” aimed to conduct an updated literature review on the clinical, diagnostic, and therapeutic aspects of premature rupture of membranes (PROM). This condition, considered a significant obstetric complication, is characterized by the loss of amniotic fluid before the onset of labor, occurring in both term and preterm pregnancies, and is associated with various maternal and fetal risks. Through a systematic review based on the PRISMA 2020 methodology, the leading causes, current diagnostic methods, and clinical management strategies were identified to support decision-making and contribute to higherquality obstetric care. PROM is classified according to gestational age, with infectious, inflammatory, and structural causes being most common. Diagnosis is based on clinical findings and complementary tests, while treatment varies with gestational age, including expectant management, administration of antibiotics and corticosteroids, and, if necessary, labor induction. Twelve studies published between 2021 and 2025 were included, all of which were sourced from recognized scientific databases. Most were recent, high-quality publications, equally distributed between Spanish and English, with systematic reviews and meta-analyses being the predominant study designs. Analysis using SPSS enabled the structured organization of data. The discussion highlights that infectious and inflammatory processes are the leading causes of PROM. Diagnosis should rely on a detailed clinical history, careful physical examination, and high-precision noninvasive tests. Regarding treatment, an individualized approach is emphasized, considering gestational age and maternal-fetal status to ensure safe interventions that reduce risks without compromising well-being. In conclusion, this review integrates updated and validated clinical knowledge, reinforcing the importance of a comprehensive, ethical, and evidence-based approach to guide clinical decisions and promote better perinatal outcomes in cases of PROM.
