Summary:ABSTRACT: Diabetes mellitus is a paradigmatic disease in older adults. Its prevalence that increases with age, the variety of forms of onset, clinical manifestations and complications, the affectation it generates on the functional capacity of patients and the decreased perception of quality of life that it conditions are some of its distinctive characteristics. Appropriate management from the diagnostic and therapeutic point of view from the onset of the disease can be decisive in its clinical evolution. The objective of this research was to update the knowledge related to the diagnostic and therapeutic management of the onset of diabetes mellitus in the elderly. For this, a basic, descriptive and cross-sectional investigation was carried out consisting of a bibliographic review of documents published in the last 5 years. A total of 109 documents were identified, of which 79 were used in the study. The information was obtained from regional (Latindex, Lilacs, Scielo, and Redalyc) and high-impact databases (Scopus, Medline, PubMed, and Ice Web of Science). The main results include the description of the debut clinical characteristics of diabetes mellitus in older adults and the elements related to the diagnostic and therapeutic management of the disease in this population group. A diagnostic and therapeutic algorithm was also designed based on the elements that allow the initial diagnosis of the disease. It is concluded that in the majority of older adults the disease debuts asymptomatically, additionally there are multiple forms of debut with clinical manifestations and therapeutic conduct that must be individualized depending on the form of debut of diabetes mellitus.
