Appropriate management from the diagnostic and therapeutic point of view from the onset of the disease can be decisive in its clinical evolution. The objective of this research was to update the knowledge related to the diagnostic and therapeutic management of the onset of diabetes mellitus in the elderly. For this, a basic, descriptive and cross-sectional investigation was carried out consisting of a bibliographic review of documents published in the last 5 years. A total of 109 documents were identified, of which 79 were used in the study. The information was obtained from regional (Latindex, Lilacs, Scielo, and Redalyc) and high-impact databases (Scopus, Medline, PubMed, and Ice Web of Science). The main results include the description of the debut clinical characteristics of diabetes mellitus in older adults and the elements related to the diagnostic and therapeutic management of the disease in this population group. A diagnostic and therapeutic algorithm was also designed based on the elements that allow the initial diagnosis of the disease. It is concluded that in the majority of older adults the disease debuts asymptomatically, additionally there are multiple forms of debut with clinical manifestations and therapeutic conduct that must be individualized depending on the form of debut of diabetes mellitus. |

Su prevalencia que aumenta con la edad, la variedad de formas de debut, de manifestaciones clínicas y complicaciones, la afectación que genera a la capacidad funcional de los pacientes y la disminución de percepción de calidad de vida que condiciona son algunas de sus características distintivas. El manejo adecuado desde el punto de vista diagnóstico y terapéutico desde el debut de la enfermedad puede ser decisivo en su evolución clínica. El objetivo de esta investigación fue actualizar los conocimientos relacionados con el manejo diagnóstico y terapéutico del debut de diabetes mellitus en los adultos mayores. Para esto se realizó una investigación básica, descriptiva y transversal consistente en una revisión bibliográfica de documentos publicados en los últimos 5 años. Se identificaron un total de 109 documentos de los cuales 79 fueron utilizados en el estudio. La información se obtuvo de bases de datos regionales (Latindex, Lilacs, Scielo y Redalyc) y de alto impacto (Scopus, Medline, PubMed e Ice Web of Sciencie). Los principales resultados incluyen la descripción de las caracteristicas clínicas de debut de la diabetes mellitus en adultos mayores y de los elementos relacionados con el manejo diagnóstico y terapéutico de la enfermedad en este grupo poblacional. También se diseñó un algoritmo diagnóstico y terapéutico basado en los elementos que permiten el diagnóstico de debut de la enfermedad.
