Therefore, in Ecuador, the Ministry of Public Health implemented tools such as the Score mom and the guide of obstetric keys through the "Maternal Alarm" strategy to prevent maternal death. In Ecuador, 55 maternal deaths due to abortion/lethality were registered during these five years. Through global and local research, it has become necessary to identify the incidence of the red key with abortion as the primary cause. This work aims to describe the incidence of red key activation as a hemorrhagic complication of abortion in patients admitted to the Gynecology and Obstetrics Service of the "Hospital General Provincial Docente Riobamba" during the year 2021. The research was a non-experimental, observational, cross-sectional study in which a group of gynecological patients was analyzed through variables designated to a group of patients, using descriptive statistical techniques. It was determined that the incidence of red key due to abortion represented 13.9% of such pathology, being other obstetric antepartum and postpartum hemorrhages. The main factor in the incidence of a red key was other antepartum and postpartum obstetric hemorrhages, which accounted for 86.1%. In the gynecological-obstetric history, it was observed that 47.1% of the patients who presented red key had had an abortion in their previous pregnancies. A significant percentage of 23.5% of patients were exposed to tobacco smoke. As for nutritional status or BMI, there was a representative group of 41.2% of overweight patients. Incomplete abortion has a high incidence in cases that developed red key with a representation of 76.4%, followed by septic abortion with 17.7%. The failure of pharmacological treatment by the sole use of misoprostol as an abortive method is the leading cause for which an abortion activates the red key protocol, with 64.7% of cases studied. The results indicated that most red key activations were for causes not inherent to abortion. A low incidence of red key activation due to miscarriage was found concerning other obstetric hemorrhages.It demonstrates that, in the General Teaching Provincial Hospital of Riobamba, the effective management of the red key by the health personnel, based on what has been investigated and supported in different works of literature where it is evidenced that the appearance of severe hemorrhagic complications during the first half of pregnancy is frequent, thus complying with the quality standards in maternal safety.
