description ABSTRACT The obstetric red key is a protocol with comprehensive management for timely identification of signs and symptoms that directly prevent future maternal complications, thus avoiding the increase of maternal death cases as a direct cause of pregnancy. Therefore, in Ecuador, the Ministry of Public Health implemented tools such as the Score mom and the guide of obstetric keys through the "Maternal Alarm" strategy to prevent maternal death. In Ecuador, 55 maternal deaths due to abortion/lethality were registered during these five years. Through global and local research, it has become necessary to identify the incidence of the red key with abortion as the primary cause. This work aims to describe the incidence of red key activation as a hemorrhagic complication of abortion in patients admitted to the Gynecology and Obstetrics Service of the "Hospital General Provincial Docente Riobamba" during the year 2021. The research was a non-experimental, observational, cross-sectional study in which a group of gynecological patients was analyzed through variables designated to a group of patients, using descriptive statistical techniques. It was determined that the incidence of red key due to abortion represented 13.9% of such pathology, being other obstetric antepartum and postpartum hemorrhages. The main factor in the incidence of a red key was other antepartum and postpartum obstetric hemorrhages, which accounted for 86.1%. In the gynecological-obstetric history, it was observed that 47.1% of the patients who presented red key had had an abortion in their previous pregnancies. A significant percentage of 23.5% of patients were exposed to tobacco smoke. As for nutritional status or BMI, there was a representative group of 41.2% of overweight patients. Incomplete abortion has a high incidence in cases that developed red key with a representation of 76.4%, followed by septic abortion with 17.7%. The failure of pharmacological treatment by the sole use of misoprostol as an abortive method is the leading cause for which an abortion activates the red key protocol, with 64.7% of cases studied. The results indicated that most red key activations were for causes not inherent to abortion. A low incidence of red key activation due to miscarriage was found concerning other obstetric hemorrhages.It demonstrates that, in the General Teaching Provincial Hospital of Riobamba, the effective management of the red key by the health personnel, based on what has been investigated and supported in different works of literature where it is evidenced that the appearance of severe hemorrhagic complications during the first half of pregnancy is frequent, thus complying with the quality standards in maternal safety. Keywords: red key, abortion, obstetric hemorrhage, curettage.RESUMEN La clave roja obstétrica es un protocolo con un manejo integral para identificación oportuna de una serie de signos y síntomas que previenen directamente futuras complicaciones maternas evitando así el aumento de casos de muerte materna como causa directa del embarazo. Por lo que, en Ecuador el Ministerio de Salud Pública implemento herramientas como el Score mamá y la guía de claves obstétricas por medio de la estrategia “Alarma materna” para la prevención de muerte materna. En el Ecuador durante estos cinco años se registraron 55 muertes maternas a causa de un aborto/legrado. Por medio de investigaciones a nivel global y local, se ha visto necesario identificar la incidencia de clave roja teniendo al aborto como causa principal. Objetivo General: Describir la incidencia de la activación de clave roja como complicación hemorrágica del aborto en pacientes ingresadas en el Servicio de Ginecología y Obstetricia del Hospital General Provincial Docente Riobamba, durante el año 2021. Material y métodos: Estudio no experimental, observacional, de corte transversal en la cual se desarrolla por medio de análisis de variables designados a un grupo de pacientes obstétricas, utilizando técnicas de estadística descriptiva. Resultados: se determina, la incidencia de clave roja por aborto representó el 13,9% de dicha patología, siendo otras hemorragias obstétricas anteparto y postparto. Las que ocupan una representación mayor 86,1% como factor principal de incidencia de clave roja. En los antecedentes gineco-obstétricos se observó que el 47,1% de pacientes que presentaron clave roja, en sus anteriores gestas habían presentado un aborto. Se constata un porcentaje significativo del 23,5% de pacientes que están expuestas al humo del tabaco, en cuanto al estado nutricional o IMC, se observa un grupo representativo de pacientes con sobrepeso del 41,2%. El aborto incompleto tiene gran incidencia en cuanto a los casos que desarrollaron clave roja con una representación del 76,4%, seguido del aborto séptico con un 17,7%, se evidencia que el fracaso de tratamiento farmacológico por el uso único del misoprostol como método abortivo, es la principal causa por la que un aborto activa el protocolo clave roja con el 64,7% de casos estudiados. Conclusiones: Los resultados indicaron que activaciones de clave roja fueron por causas no inherentes al aborto. Determinando una baja incidencia de activación clave roja por aborto, en relación a otras hemorragias obstétricas.
