General objective: To establish the factors associated with morbidity and mortality in HELLP syndrome in pregnant women with Obstetric High Risk at the Obstetric service of the Carlos Andrade Marin Hospital from May 2020 to May 2021. Materials and methods: A retrospective, descriptive, non-experimental, and cross-sectional study was carried out. For this study's purpose, the researcher analyzed 30 clinical histories of pregnant women diagnosed with HELLP Syndrome at the Obstetric High-Risk Obstetrics Service of the Carlos Andrade Marín Hospital from May 2020 to May 2021. The researcher used the quantitative-qualitative method. Results: Of the 30 cases studied, it was determined that 76% were within the age range of 30-45 years, in addition to being of mixed race (90%), multigestational (63%), with early gestational age (37%), singleton pregnancy (97%), with a history of hypertensive disorders (40%). Of these, the main complications in these patients were related to IUGR (23%) and acute renal failure (20%) with a morbidity ratio of 13.83% and mortality rate of 0% as there were no deaths within the period studied. Conclusions: The higher incidence of HELLP syndrome is associated with the previous history of preeclampsia, in addition to an absence in the use of acetylsalicylic acid, which prevents preeclampsia and its complications. Objetivo general: Establecer los factores asociados a la morbimortalidad en el síndrome HELLP en gestantes del servicio de Alto Riesgo Obstétrico – Obstetricia del Hospital Carlos Andrade Marín en el periodo mayo 2020 – mayo 2021. Materiales y métodos: Se realizo un estudio retrospectivo, descriptivo, no experimental y de corte transversal en el cual se analizaron 30 historias clínicas de gestantes con diagnóstico de Síndrome HELLP en el Servicio de Alto Riesgo Obstétrico – Obstetricia del Hospital Carlos Andrade Marín desde mayo 2020 a mayo 2021, mediante el uso del método cuantitativo – cualitativo. Resultados: De los 30 casos estudiados, se determinó que el 76% se encontraban dentro de los rangos de edad que abarca 30 – 45 años, además que eran mestizas (90%), multigestas (63%), con edad gestacional precoz (37%), embarazo simple (97%), con antecedentes de trastornos hipertensivos (40%). De estos, las principales complicaciones que se presentaron en estas pacientes se relacionan con RCIU (23%) y la insuficiencia renal aguda (20%) con una tasa de morbilidad del 13.83% y mortalidad del 0% al no presentarse defunciones dentro del periodo estudiado.
