Objective, to perform an updated review of the Step-Up strategy and its efficacy in the management of PAN in order to improve the prognosis of this disease. Methodology a qualitative approach was applied in this study, carrying out a literature review in academic databases such as “Pubmed” “Google Scholar” and “MedScape”, retrospective articles, focused on patients with PAN were selected for analysis. Result of the 35 studies analyzed, 8 were included in the analysis. A significant portion was found to support the use of minimally invasive interventions as a first option before considering open necrosectomy. The Step-Up strategy was shown to offer significant advantages in a considerable number of cases, while the 4-step approach showed effectiveness in a smaller proportion, reducing risks and complications. However, open necrosectomy led to major complications in a minority of cases. Conclusions we suggest initiating treatment with conventional measures and antibiotics, considering the Step-Up strategy at a later stage and contemplating open necrosectomy in cases of failure, while maintaining caution due to its potential complications. This review underlines the importance of a gradual and personalized approach in the management of PAN to improve clinical outcomes and reduce associated mortality. |

Objetivo realizar una revisión actualizada de la estrategia Step-Up y su eficacia, en el manejo de la PAN, con el fin de mejorar el pronóstico de esta enfermedad. Metodología se aplicó un enfoque cualitativo en este estudio, llevando a cabo una revisión bibliográfica en bases de datos académicas como “Pubmed” “Google Académico” y “MedScape”, se seleccionaron artículos retrospectivos, centrados en pacientes con PAN para su análisis. Resultados de los 35 estudios analizados, se incluyeron 8 en el análisis. Se encontró que una parte significativa respalda el uso de intervenciones mínimamente invasivas como primera opción, antes de considerar la necrosectomía abierta. La estrategia Step-Up demostró ofrecer ventajas significativas en un número considerable de casos, mientras que el enfoque de 4 pasos mostró efectividad en una proporción menor, reduciendo riesgos y complicaciones. Sin embargo, la necrosectomía abierta conllevó mayores complicaciones en una minoría de casos. Conclusiones se sugiere iniciar el tratamiento con medidas convencionales y antibióticos, considerar la estrategia Step-Up en una etapa posterior y contemplar la necrosectomía abierta en casos de fracaso, manteniendo precaución debido a sus potenciales complicaciones.
