This was a study of descriptive, comparative, retrospective and cross-sectional case-control consists of two groups, an adolescent group (10-19 years) with 910 cases and a control group (20-29 years) with 2261 cases they were treated at the Gynecology and Obstetrics Service at the Regional Hospital Isidro Ayora during the period January - December 2013; information from medical records before developing a data collection sheet was obtained. For subsequent statistical analysis Chi Square formula is applied, then the results were expressed in tables and charts (bars and cakes), with the following results: the most common complications in the group of adolescents during pregnancy are: preterm labor 9.23%, tract infection urine 8.68%, anemia 3.62%, where there was a highly significant difference. In complications such as incomplete abortion 8.79%, threatened abortion 3.85%, missed abortion 3.29%, preeclampsia 2.53% and premature rupture of membranes 2.09%, were more frequent in adolescents with a significant difference. Unlike the pregnancy induced hypertension were in higher percentage in adult stem 1.11% with no statistical difference. Complications during labor predominated in the group of adult mothers were: perineal tear 6.72%, cesarean 21.94% delivery and prolonged labor only 0.99% was more common in adolescents. Finally neonatal complications were more common in teenage mothers as 1.87% birth asphyxia, low birth weight and IUGR 0.33%, SDR type I and prematurity 0.11%, congenital malformations 0.88%, and stillbirth 1.43%. KEYWORDS: Teen Pregnancy, Maternal Complications, Neonatal Complications.En el presente trabajo se pretendió determinar las complicaciones más frecuentes durante el embarazo, parto y nacimiento en adolescentes embarazadas al comparalas con madres en edad reproductiva óptima. Se trató de un estudio de tipo descriptivo, comparativo, retrospectivo y de corte transversal de caso y control conformado por dos grupos, un grupo adolescente (10 a 19 años) con 910 casos y un grupo control (20 a 29 años) con 2261 casos que fueron atendidas en el servicio de Gineco-Obstetricia en el Hospital Regional Isidro Ayora durante el periodo Enero – Diciembre del 2013; se obtuvo la información de las historias clínicas previo la elaboración de una hoja de recolección de datos. Para su posterior análisis estadístico se aplicó la fórmula de Chi Cuadrado, luego se plasmaron los resultados en cuadros y gráficos (barras y pasteles), obteniendo los siguientes resultados: las complicaciones más frecuentes en las adolescentes durante el embarazo son: amenaza de parto pretérmino 9.23%, infección del tracto urinario 8.68%, anemia 3.62%, donde hubo una diferencia altamente significativa. En complicaciones como: aborto incompleto 8.79%, amenaza de aborto 3.85%, aborto diferido 3.29%, preeclampsia 2.53% y ruptura prematura de membranas 2.09%, fueron más frecuentes en las adolescentes con una diferencia significativa. Al contrario la hipertensión inducida por el embarazo se presentó en mayor porcentaje en las madres adultas 1.11% sin diferencia estadística. Las complicaciones durante el parto predominaron en el grupo de madres adultas y fueron: desgarro perineal 6.72%, parto por cesárea 21.94% y únicamente el parto prolongado 0.99% fue más frecuente en las adolescentes. Por último las complicaciones neonatales fueron más frecuente en las madres adolescentes como:asfixia neonatal 1.87%, bajo peso al nacer y RCIU 0.33%, SDR tipo I y prematurez 0.11%, malformaciones congénitas 0.88%, y óbito fetal 1.43%.
