Summary:Worldwide, acute appendicitis is one of the most common causes of abdominal pain and a frequent cause of surgical intervention in emergency departments. However, overlap with other symptoms can complicate its diagnosis and delay its treatment, increasing the risk of postoperative complications. The present research work had the following objectives: to determine the risk factors associated with postoperative complications, to determine which risk factor is more prevalent in patients, to identify the main postoperative complications and to determine the degree of acute appendicitis associated with the type of postoperative complication in patients of the General Hospital Isidro Ayora-Loja during 2019-2021 period. A descriptive, quantitative, retrospective, cross-sectional study was carried out on a sample of 64 patients aged 20-60 years old. Results: The most common risk factor was self-medication (82.8%). The most common postoperative complications were surgical site infection (37.5%), intra-abdominal abscess (17.2%), enterocutaneous fistula (15.6%), appendiceal stump dehiscence (9.4%), paralytic ileus (7.8%), atelectasis (4.7%), pneumonia (3.1%) and bleeding (1.6%). The severity of perforated appendicitis (50%) was associated with most postoperative complications, and there was a moderate correlation between the severity of appendicitis and postoperative complications (r=0.412; p<0.05). In conclusion, self-medication is a risk factor for the development of postoperative complications, with surgical site infection being the most common, followed by intra-abdominal abscess and enterocutaneous fistula, and patients with later grades of appendicitis (perforated) are associated with postoperative complications
