| Samenvatting: | The completion of the research work contributes to strengthening the comprehensive care model of the National Intercultural Community and Family Health at the University Hospital of Motupe , providing statistics and data that enable basic health teams develop local proposals aimed at improving the quality of life of the sectors I and II of the San Jacinto Neighborhood conducting a community diagnosis , epidemiological profile and talking map , as a basis for formulating the Health Development Project and provide specific nursing services , designed to ensure the exercise of right to health of disadvantaged groups as a form of community outreach and linking strategy with the health service. This is a descriptive study worked with 66 families and 296 habitants; the instrument was the family record of MSP and an annex. The population is young, with a level of primary school. There is mild under nutrition and overweight, disabled, families with poor garbage disposal, excreta and liquid intradomiciliary animals, consumption of unsafe water, environmental impact industries. Unemployment or informal employment of the household head was found engaged in masonry and trade. There is also poverty, poor housing, overcrowding, cases of alcohol and domestic violence. Health services attending both public and private. The sexually active women using fertility control methods, many women were not performed Pap. Alcoholism, diabetes and hypertension are the leading causes of morbidity and mortality. Counseling is provided in response to the needs of Community Health.
