সংক্ষিপ্ত:This research is descriptive, prospective and cross-sectional in pregnant women with a diagnosis of acute Fetal suffering (SFA), served in the area of emergency and hospital of Gynecology and obstetrics of the Hospital Provincial Isidro Ayora from the city of Loja. The main objective in the research was: demonstrate the effectiveness of the determination of resistance of fetal Middle cerebral artery and artery of cord as predictive diagnostic test of acute fetal distress and its correlation with pH of umbilical cord postpartum and Apgar score at 1 and 5 minutes of the newborn (RN). According to the proposed objectives, the results were: The more altered resistance index was 75.3% Middle cerebral artery; while the altered index of umbilical artery was 64.61%. Linking the results of the determination of the levels of resistance with postpartum umbilical artery pH results; was obtained a 95,23%, a specificity of 60,86% sensitivity and positive predictive value of 81,63% for the Middle cerebral artery; While that obtained a 92,10%, a specificity of 74,07% sensitivity and predictive value positive 83.33% for umbilical cord artery. Linking the results of the determination of the rates of resistance values for APGAR of the RN 1 minute; was obtained a sensitivity of 92.5%, a specificity of 52,0% and positive predictive value of 75.5% for middle cerebral artery; While that was obtained a sensitivity of 88.8%, a specificity of 65.5% and a predictive value positive 76,19% for umbilical cord artery. Linking the results of the determination of the rates of resistance values for RN APGAR 5 min; was obtained a sensitivity of 90%, a specificity of 27.27% and positive predictive value of 18.36% for middle cerebral artery; While that was obtained a sensitivity of 60%, a specificity of 35% and a predictive value positive 18.17% for umbilical cord artery. Key words: acute fetal distress, resistance indexes, pH, Apgar.
