Summary:This study type was descriptive, retrospective and transversal cut, the purpose is to identify the principal causes of emergent exploratory laparotomy in patients with traumatic and nontraumatic acute abdomen in the Manuel Ygnacio Monteros hospital of Loja, the specific objectives were, to stablish the pre and postsurgical diagnoses by was done emergent exploratory laparotomy in patients with traumatic and nontraumatic acute abdomen, to determinate the contribution of image studies at the postsurgical diagnose. This study was done with 251 patients in total that comply the inclusion criteria, the information was obtained through a date card record, based at the clinic histories of AS400. The most frequent cause of laparotomy in patients with acute abdomen was nontraumatic origin (84,46%) which principal etiologies was inflammatory (51,82%), at the traumatic origin was close abdominal trauma (84,62%). The presurgical diagnose the predominant traumatic abdomen was haemoperitoneum (35,90%) and the postsurgical haemoperitoneum with perforation of hollow viscus (23,08%). The pre and postsurgical in nontraumatic acute abdomen excelled was acute appendicitis 29,25% y 23,58% respectively. The image method that contribute in traumatic and nontraumatic acute abdomen is Computarized Axial Tomography with a positive predictive value of 91,8% and negative of 81,2%. Concluding the principal cause of exploratory laparotomy is the nontraumatic acute abdomen which diagnoses pre and postsurgical had inflammatory cause, the image method more contributor in postsurgical diagnose is the CAT followed by Eco Fast and Echography.
