Sumario:Leishmaniasis is a major public health problem in endemic areas due to its wide distribution and prevalence. While effective treatment options with high cure rates are available, access to these treatments often requires extensive travel, financial expenses, and daily visits to healthcare centers. These challenges create substantial disruptions in the lives of affected individuals, leading some to abandon treatment altogether. The objective was to analyze the socioeconomic, cultural, and health-related factors, the rate of treatment abandonment, and the strategies implemented to avoid it. The study was conducted as a systematic review, structured using the PICO framework, and guided by the PRISMA methodology. The JBI assessment tool was employed to evaluate risk, and results were presented in a summary table. Key socioeconomic and cultural factors associated with treatment discontinuation included low socioeconomic status, high medication costs, reliance on home remedies, and residing in remote areas far from treatment centers. Reported dropout rates ranged from 2.4% to 14.2%, with some cases as high as 22.4%. Strategies to improve adherence included the use of mobile applications for monitoring treatment progress and side effects, as well as the development of new, more easily administered medications. At the end of the present investigation, it was found that the treatment of leishmaniasis is complex and one of the major barriers or factors associated with treatment abandonment are the economic limitations and cultural beliefs of the most affected populations
