Summary:The acute appendicitis is considered as a common health problem of acute surgical abdomen, occupying the third cause of morbidity in Ecuador and a mortality of 2,4 thousand appendectomies. Due to this, it was carried out the present study using a descriptive, quantitative, retrospective and transversal methodology; which objectives were to establish the frequency of patients with acute appendicitis that attend to the “Hospital Regional Isidro Ayora” for the emergency service and surgery; the second objective was to determine the general features of the group study: age and sex, in patients with diagnosis of complicated acute appendicitis; the third objective was to identify the factors such as self-medication, diagnosis error in the initial consultation, time of evolution and reference sheet as a cause of delaying in the diagnosis and treatment of complicated acute appendicitis and the fourth objective was to establish the frequency of complications in patients that have been surgically intervened with diagnosis of complicated acute appendicitis. The sample consisted of 53 patients that accomplish with the inclusion criteria; obtaining as results the frequent ages between children of 5 and 13 years, and adults of 36 and 64 years with 25%, predominating in the female gender, 17% had diagnosis error in the initial consultation: the time elapsed from the beginning of symptoms until the surgery is 73 hours with 45%, concluding that the main cause that delays the diagnostic and treatment is the self-medication and the most common complication is the peritonitis located with 57%.
