| Streszczenie: | Acute cholecystitis is an acute inflammation of the gallbladder secondary to a mechanical, chemical or bacterial cause, being a worldwide health problem, where its therapeutic success depends on an early diagnosis that can be made by applying clear and simple diagnostic criteria. The proposed objective is to determine Tokyo Guidelines’s criteria correlation with the histopathological report in the diagnosis of acute cholecystitis in patients that underwent cholecystectomy at the Manuel Ygnacio Monteros´s Hospital-IESS Loja. The instrument used was a data collection form that contains general data such as sex, age, and diagnostic criteria based on the signs and symptoms of each patient. This is a retrospective, descriptive study, which was carried out with a sample of 127 patients diagnosed with acute cholecystitis at the Manuel Ygnacio Monteros´s Hospital-IESS Loja during 2018, whose information was obtained from the medical records. Obtaining as results that the patients with the highest prevalence were those aged between 30 and 45 years, with 32.58%, being the female gender the most affected. Diagnosis through the Tokyo Guidelines 2018 was positive in 45.67%, for acute cholecystitis, while the result of the histopathological examination was negative in 54.33%, of all patients. The Tokyo Guidelines had a sensitivity of 80.36% and a specificity of 81.69%. Also, a positive predictive value of 77.59% and negative predictive value of 84.06%.
