Summary: | Infectious mononucleosis (IM) is a systemic disease produced 90% of times by the Ebstein-Barr virus (EBV) that infects 95% of the population. It is characterized by: fever, adenomegalias, pharyngotonsillitis and, occasionally, splenomegaly and rash, especially after taking ampicillin. Other germs (cytomegalovirus, toxoplasma, rubella and HIV). In young children, primary infection is earlier and often asymptomatic or presents nonspecific symptoms. In developed countries, infection is delayed, and adolescents and adults have infectious mononucleosis. Man is the only source of contagion and the main transmission is saliva. The most common local complication is bacterial pharyngoamygdalar superinfection. Splenic rupture is serious but rare. The diagnosis of infectious mononucleosis is based on: clinical symptoms, haematological findings [leukocytosis with lymphomonocytosis (> 50%), atypical lymphocytes (> 10%) and increase in liver enzymes] and microbiological tests. The presence of heterophile antibodies (AH) is diagnostic. In the case of suspicion, but negativity of AH, the specific serology is used. There is no specific treatment for infectious mononucleosis. Corticosteroids could be used in serious complications. Infectious mononucleosis is a benign and self-healing disease in 3-4 weeks. |
