| Summary: | This clinical case is about a 44-year-old patient from the city of Guayaquil, her definitive diagnosis was thyroid nodule (E04.1), with a personal pathological history of rheumatoid arthritis diagnosed 1 year ago. Signs of nutritional deficiency were evidenced by physical examination such as dry skin, fine and dull hair, and the patient was overweight with a body mass index of 26.7kg / m2, hypercholesterolemia (250 mg / dl), Lipoproteins Low density or LDL (150mg / dl), hypertriglyceridemia (385mg / dl), related to an inadequate diet, high in simple carbohydrates and saturated fats. It should be mentioned that the thyroid stimulating hormone (TSH) was also high (6 mIU / L), indicating a dysfunction of the thyroid gland. A dietary-therapeutic plan of 1487.3 kcal / day divided into 4 meal times was implemented, with which the nutritional status was improved throughout the three months of treatment, evidenced in more hydrated skin, better hair appearance, a BMI of 24.6kg / m2 (normal), a weight loss of 7.6% (adequate), total cholesterol was reduced to 198mg / dl (adequate), low density lipoproteins at 129mg / dl, triglycerides at 145mg / dl ( suitable). Thyroid Stimulating Hormone (TSH) improved to 2.2mUI / l (normal). The possibility of thyroid cancer was ruled out by a biopsy indicated by the attending physician. |
