| Περίληψη: | The present study is focused on the pathology of idiopathic thrombocytopenic purpura that affects the immune system and is characterized by platelet dysfunction causing increased bleeding due to its clotting time. Among the most predominant clinical manifestations are mucocutaneous hemorrhages and the appearance of petechiae in any part of the body. This disease is treatable and with appropriate treatment the patient can have a full recovery. However, the following clinical case study is applied to a 19-year-old female patient at 22 weeks of gestation who presented a clinical picture of frank hematuria, disseminated petechiae on the trunk and extremities, asthenia and generalized mucocutaneous pallor. More with hematoma in the right parieto-occipital region, ecchymosis in the oral mucosa and sclera of the right eye. Vital signs are taken, showing blood pressure 130/82 mmHg, heart rate 98lxmin, respiration 22rxmin, temperature 36.6º C, saturation 98%, weight 72.5kg and height 157cm, breast score 0. Peripheral line is placed and Ringer's lactate 1000ml IV 21ml/h, methylprednisone and omeprazole were administered. Laboratory tests, fetal Doppler and weekly ultrasound are performed to evaluate the maternal-fetal status. The test results reported mild anemia, platelets of 2,000mm3/µL. It was decided to transfuse 7 units of platelets and treatment with glucocorticoids was started. The nursing assessment is performed using Marjorie Gordon's functional patterns to identify dysfunctional patterns that compromise maternal-fetal health. And proceed to apply the Nursing Care Process with the objective of providing the optimal care that the patient requires during her hospital stay, and thus meet the objectives proposed in this case study. |
