Resumo:Postpartum hemorrhage is considered one of the major obstetric complications, since its risk factors imply a high incidence of maternal morbidity and mortality. Universally it is defined as the blood loss greater than 500 ml after a vaginal delivery or 1,000 ml after a cesarean delivery. Primary postpartum hemorrhage is that which occurs during the first 24 hours after delivery, and is usually the most severe and late secondary hemorrhage occurs after 24 hours after delivery. The causes of postpartum haemorrhage include atony, traumas / lacerations, retention of products of conception and alterations in coagulation, the most frequent being atony. The diagnosis is based on the clinic of the patient, the treatment is performed based on the degree of involvement in the mnemonic of the four T, which means at the level of Tone, Trauma, Tissue, Thrombin. Within the treatment, uterotonics, such as ergometrine, oxytocin, are the first choice, and the bakri balloon is also used if the pharmacological treatment does not work, and when the patient does not intend to undergo pharmacological or surgical treatment. A 19-year-old patient with post-partum haemorrhage was studied in the present clinical case. She reported a clinical picture of approximately 24 hours of evolution, which was immediately treated with the respective treatments based on uterotonics and the blood decompensation that I present transfusion of two globular units was performed to compensate for his hemodynamic condition, which the patient benefited beneficially to the treatment.
