Resumo:The objective of this case study is to develop a nursing care plan with the benefit of improving and reducing health problems in an older adult patient diagnosed with vascular dementia. Vascular dementia is a disorder of the brain that is characterized by loss and difficulty of memory is caused by problems with the blood vessels that feed the brain and is the second cause of dementia in older adults after Alzheimer's disease and represents 10% -20% of cases of dementia, in the early stages, this disorder causes cognitive difficulties with reasoning and discernment, in later stages, affects memory. The profile of the patient with vascular dementia, and the neurological alterations that will accompany the evolution of this, is very heterogeneous since it depends on the location and extent of the lesion, cerebrovascular diseases will cause an alteration in the blood supply to the brain, giving rise to focal brain damage, that is to say specific, or to a more global and diffuse effect, known as diaschisis, an adequate nursing plan executed to the condition presented by the patient will help the recovery and improvement of the patient .
