| Tóm tắt: | Pneumonia is an infection of the lung parenchyma that occurs when a pathogen, such as bacteria, viruses, or fungi, invades lung tissue. The individual's immune system may be compromised by various predisposing conditions, such as smoking, chronic lung diseases such as COPD, alcoholism, malnutrition, and other factors, which facilitate the entry of these microorganisms into the lower respiratory tract and trigger an infectious process. It is essential to consider risk factors in the prevention of pneumonia, especially in the geriatric population, since older adults are at greater risk of developing serious infections and associated complications. The functional health pattern model proposed by Virginia Henderson was used to structure and guide the patient's assessment, thus allowing a comprehensive understanding of her health status. The implementation of an individualized care plan, based on the categories NANDA (Nursing Diagnoses), NIC (Nursing Interventions) and NOC (Nursing Outcomes), has demonstrated significant improvements in treatment compliance, general health status, emotional well-being, nutritional knowledge and adequate symptom monitoring. The purpose was to achieve the medical discharge of a geriatric patient diagnosed with community-acquired pneumonia, through specific nursing interventions that favored the improvement of the health status during her hospitalization in the internal medicine unit. |
