Ajouter un tag | Résumé: | The Pharmaceutical care is a service for the patient with the responsibility of the pharmacist. The work of the pharmacists is fundamental since they ensure access to the drug in coordination with the doctor consequently it is a professional work to support to the patients, helping to ensure the effectiveness and safety of medicines. The objective was to make the pharmaceutical care in a period of three months. We used the Dader method for pharmaceutical care that aims to help needs of the patients. We visited the 55 patients in your home, where we made the first interview. As a result some problems related to medicines were detected: 5% was for Need, 2% for Effectiveness, and 7% for Safety. Finally we made the intervention, on nutrition, physical activity for hypertension and the role of the family with health problems. |
