“Evaluación de la microfiltración en restauraciones indirectas cementadas con resina precalentada, cemento de grabado total y un agente auto adhesivo”

Description

The purpose of this study was to evaluate microfiltration in indirect restorations cemented with RelyX ARC 3M that is a total etch dual cement; with RelyX U 200 3M that is a dual self-adhesive cement and with nanoparticle preheated resin A1 Body Filtek Supreme Z 350 3M at 55°. The study was made with 60 third molars teeth, in which cavities were made in mesial oclusal and distal part with a measure of 4 mm of thickness and 4 mm of wideness (inlays). These cavities were imprinted with a Zhermack heavy and light addition silicone, and the indirect material to restore these cavities was a laboratory composite (A1 Body Ceramage of the Shofu). The samples were divided into three groups of 20, and so in the first group the cement used was the total etch cement RelyX ARC; in the second group the cementation agent was the self-adhesive cement RelyX U200 and in the third group the cement used was the preheated resin to 55̊C. All samples were photocured with halogen light Optilux 501 from Kerr. The samples cemented with RelyX U200 and with preheated resin were photocured for 20 seconds for each face as the commercial house (maker) indicates; while the samples cemented with RelyX ARC were photocured for 40 seconds for each face as the commercial house indicates. After cementation, the samples were aged with thermal cycling for 10000 cycles which means a year of use of the indirect restauration inside the mouth. In the thermal cycling the temperatures went from 4 degrees that was the coldest temperature that a human being can tolerate inside the mouth and to 56 degrees that was the hottest temperature that a human being can tolerate inside the mouth. The thermo cycle also included 35 degrees in the middle that is the temperature that a human being has inside the mouth when it is close. Then the samples were immersed in 2% basic fuchsin solution for 24 hours at room temperature. After been all the samples submerged in the die solution, macrophotograpy was taken with a digital camera and a macro lens of 100 mm, from its oclusal mesial and distal surface; to evaluate the percentage of microleakage between dental structure and the inlay made of a laboratory composite. The evaluation went between ranges of 0 till 4, were 0 was when there were any marginal pigmentation, while 4 was the highest quantity of marginal microfiltration. From this results it can be concluded that the samples with the lowest marginal microfiltration were cemented with preheated resin which had results stadistically significant compared with the other two cements. The agent that followed the pre heated resin was RelyX ARC that is a total etch cement. Finally, the samples with the highest marginal microfiltration were the ones cemented with the self-adhesive cement RelyX U 200.
El propósito de este estudio fue evaluar la microfiltración en restauraciones indirectas de cerómero cementadas con un cemento dual de grabado total RelyX ARC de la 3M; con un cemento dual autoadhesivo RelyX U 200 de la 3M, y con resina precalentada nanoparticulada a 55°C Filtek Supreme Z 350 A1 Body de la 3M. La muestra estuvo constituida por 60 terceros molares, en los cuales se confeccionó cavidades mesio-ocluso-ditales, con 4 mm de profundidad y 4 mm de ancho (inlays). A dichas cavidades se les tomó impresión con una silicona de adición pesada y liviana marca Zhermack y para ellas se elaboró incrustaciones de cerómero (A1 Body Ceramage de la Shofu). A las muestras se las dividió en tres grupos de 20; al primer grupo se le cementó las restauraciones indirectas con el cemento de grabado total, al segundo con el cemento auto adhesivo y al tercero con la resina precalentada a 55̊C. A todas las muestras se las fotocuró con una lámpara de luz halógena Optilux 501 de la Kerr; a las cementadas con RelyX ARC y resina precalentada se las fotocuró por 40 segundos por cada superficie como indica el fabricante; mientras que las muestras cementadas con RelyX U200 se las fotocuró por 20 segundos por cada cara como indica el fabricante. Posterior a la cementación las muestran fueron sometidas a 10.000 ciclos de termociclado (equivalencia de un año), en el cual se las sometió a cambios bruscos de temperatura tolerables en la cavidad bucal, siendo 4̊ la temperatura más fría, 56̊ la más caliente y 35̊ la temperatura estable al cerrar la boca. Seguido al envejecimiento de las muestras, a las mismas se las sumergió en una solución básica de fucsina al 2% por 24 horas a temperatura ambiente. Después de extraer las muestras de la sustancia pigmentante, a las mismas se les tomó macro fotrografías con una cámara digital y un lente macro de 100 mm, de su superficie oclusal mesial y distal. Se evaluó la microfiltración marginal en rangos del 0 al 4 entre la incrustación y la estructura dental de cada una de las superficies mesiales, distales y oclusales; siendo 0 el que no presenta pigmentación alguna y 4 el grado de mayor microfiltración. Concluyendo que las muestras que poseen menor microfiltración marginal son las cementadas con resina precalentada, ya que presentan una diferencia estadísticamente significativa en relación a los otros dos cementos. Seguido de la resina precalentada, los menores valores de microfiltración los presentó el agente dual de grabado total RelyX ARC; mientras que las muestras que mayor microfiltración marginal presentaron fueron las cementadas con el cemento dual auto adhesivo RelyX U 200.
