La leucemia se origina en las células primitivas productoras de sangre de la médula ósea. En la leucemia se producen síntomas y signos secundarios a insuficiencia medular. La confirmación diagnóstica se realiza con la presencia de blastos >20% en médula ósea. (1,2) El caso clínico a analizar se trata de un paciente de 5 años, residente de Guaytacama, con antecedente personal de infecciones respiratorias a repetición. Acude a esta casa de salud por presentar desde hace aproximadamente 15 días perdida súbita de peso, alza térmica continua, escalofrió y dolor óseo de miembros inferiores. Concomitantemente a este cuadro se aprecia masas no dolorosas localizadas en región submaxilar bilateral, por lo que acude a Subcentro de Salud del MSP donde se le prescribe tratamiento con antitérmicos sin efecto favorable, busca ayuda en el Hospital IESS Latacunga donde se le prescribe tratamiento con antitérmicos y amoxicilina por supuesta infección respiratoria alta, al no tener respuesta favorable, acude a médico particular quien decide remitir a esta casa de salud, al momento de su ingreso niño febril, álgico, masas a nivel submaxilar bilateral de 3 x 4 cm, pequeñas cadenas no significativas en ingles, precordio normal, se aprecia soplo holosistolico GIII en foco mitral y tricuspideo, médico tratante ingresa a paciente para realización de exámenes complementarios para su diagnóstico y tratamiento.
